Enrollment Form: 2012-2013

Please complete a separate form for each child to be enrolled in the program and enclose the fee for one year with your enrollment.

General Enrollment
$175 for one child, $100 for each additional child in the same family.

Sacrament Fees
$30 for each child who will receive First Reconciliation & First Eucharist this year; $30 for each child who will be confirmed this year. Attach a copy of a Certificate of Baptism, if not previously submitted.

Classes are held Sunday mornings in the second floor classrooms from 9:45-10:45 AM. First class will be September 9, 2012.

Sunday mornings should be set aside for family faith formation. Your commitment to your child’s religious education speaks volumes about setting priorities. Please keep our mass and class schedules in mind when deciding whether or not to sign your child up for sports or other activities that take place on Sunday mornings.
