Dear Parents,
We are delighted that you and your child will be part of the Old St. Mary’s community!
As we look ahead to the 2009-2010 school year, we have enclosed some information that will be useful to you.
• School Calendar
You will receive a completed calendar in the School Directory which is published in September and given to each family. School begins at 8:30. The Playground gates are open at 8:10 to begin accepting the arrival of students. The school day ends at 2:30.
• Medical, Dental & Vision Forms
All children will need to have a physical and health examination by a physician before (or within one year prior) entering Preschool, Kindergarten, or if they are new to the school. As many pediatricians become very busy just before the start of the school year, it is a good idea to make your appointment now.
All children entering Preschool, Kindergarten and second grade must have a dental exam. Parents are encouraged to have all of their children undergo regular dental examinations.
Effective January 2008, Public Act 95-671 requires that all children enrolling in school shall have an eye examination.
All completed medical, dental and vision forms should be mailed to the school office:
Old St. Mary’s School
1532 S. Michigan Avenue
Chicago, IL 60605
• Enrollment in the Extended Care Program
For planning purposes, complete this form if you are considering using the Extended Care Program. Children may be dropped off at school as early as 7:30 AM if required. After care is offered from dismissal (2:30) until 5:30 PM. You will be billed every two weeks for the hours you use this service. Children must be registered before accessing this service.
• Emergency Form
Please fill out this form carefully because, in the event of an emergency, we rely on it to locate you. Please list other members of your family or close friends and neighbors that we might call in the event that child should become ill at school.
• Uniform Information
Our current uniform policy states that boys will wear a white shirt and navy blue pants (navy shorts in warm weather) and girls will wear a white shirt with navy skirt, jumper, or pants (navy shorts in warm weather). These uniforms must be purchased through Land’s End or French Toast; the specific styles that Old St. Mary’s students will be allowed to wear will be available online at the end of June. You will need to enter a source code when placing your order: Land’s End – 900127085 and French Toast – QS5GWMZ. Every purchase you make at Land’s End or French Toast will directly benefit our school.
Preschool students are expected to be in uniform every day as well, but they also are allowed to wear Old St. Mary’s sweats (with school logo) and the teachers strongly encourage the children to wear them every day. They are rugged and designed for the type of play that occurs in preschool. These will be available for purchase only through Land’s End.
All students may wear Old St. Mary’s t-shirts (with school logo) on Fridays. These, too, are available only through Land’s End. If a child wishes to wear a sweater in school, they may purchase a navy blue cardigan from our French Toast web site. There is a separate style for boys and girls.
Girls may wear white or navy blue tights. All children must wear gym shoes or sensible, dark colored, walking shoes. Sandals, flip-flops, Crocs, and clogs will not be allowed.
If you have any additional questions or concerns, please do not hesitate to contact us. We will be sending out an additional mailing to you in mid-August. May you and your family enjoy a wonderful summer!
